Deadline for application: May 25, 2012.

Date of Examination:  June 12, 2012 

Requirements:

• BIRTH CERTIFICATE issued by the NSO or Local Civil Registrar duly authenticated (Original and 2 photocopies)
• ID PICTURES size 1″x1″, two identical, recently taken and not scanned Id pictures with image proportionate to the size.
• SCHOOL RECORDS (Original and 2 photocopies)

Elementary Level: Form 137 or form 138

Secondary Level:  Form 137 (with the seal of the school and signature of School Head)

• SCHOOL PERMIT photocopy of School Permit to Operate/Government Recognition (for applicants from Private School)

THERE WILL BE NO FEE FOR THE APPLICATION
